The Care Partner – Palliative Care will work with individuals, their supporters and community teams to provide optimal palliative and end of life care for older people in the community.
Care Partner – Restorative Care
The Restorative Care Partner provides care within an intensive short-term period to assist participants to regain their abilities and manage changing age-related conditions, prevent the need for higher levels of care and provide education and skills to assist individuals to retain their optimal function.
Care Partner Lead
The Care Partner Lead will work with the Care Partners to onboard individuals to the appropriate Support at Home service. The Care Partner Lead plays a crucial role in ensuring the effective delivery of community services by leading, supporting, and coordinating their team while maintaining strong client relationships and program integrity
Customer Intake Partner
The Customer Intake Partner will serve as a conduit between managing a sales pipeline whilst aligning with service delivery objectives and operating processes, acting in ways that respect the dignity and privacy of all clients.
