We have a newly created role available for an experienced Advanced Practice Nurse / Nurse Practitioner in the role of Advance Practice Nurse Wound Care with Resthaven based out of our modern head office in Wayville, SA.  Your strong leadership background in high quality evidence based best practice approach for residents and clients with complex wound care needs will be highly regarded, along with strong collaborative communication skills and experience working in aged care. About the role

  • Operate as a subject matter expert in wounds
  • Provide clinical and collaborative leadership
  • Undertake specialist clinical reviews
  • Work collaboratively with site Registered Nurses to undertake wound care review and develop best practice, evidence based wound management plans
  • Keep abreast of innovation and knowledge in the field of wounds and identify emerging trends and opportunities to implement new practice
  • Monitor wound data and reports to proactively identify any emerging trends and suggest continuous improvement plans and identify opportunity for education.
  • Manage and assess risk within areas of responsibility
  • Participate in the maintenance and reporting of Clinical Governance program and systems across Resthaven Residential and Community Services.

 

Essential criteria:

  • Current registration as a registered nurse with APHRA
  • Understanding of clinical governance.
  • Advanced wound care and continence management skills and knowledge
  • Significant experience in the management of complex skin integrity issues including pressure injuries, chronic wounds and leg ulcers, burns, Incontinence Associated Dermatitis (IADs), moisture associated skin damage, and skin tears.
  • Assessing the older person’s skin focusing on skin injury prevention strategies.
  • Educating the older person and other clinical care staff on injury prevention, wound care, skin tears, infection prevention and control, antimicrobial stewardship, effective pain assessment and management, and pressure injury prevention, staging and care
  • Clinical experience in aged care setting
  • Knowledge of and commitment to risk management, continuous improvement and quality management systems.
  • Knowledge of contemporary wound care and continence practice
  • Ability to communicate effectively both verbally and in writing with a ranged of stakeholders
  • Demonstrated experience in clinical leadership including using effective communication and interpersonal skills
  • Ability to communicate effectively with senior management and program managers and work as part of a team
  • Demonstrated ability to source clinical information, protocols and guidelines
  • Comply with all reporting requirements including incident and Serious Incident Response Scheme (SIRS) reporting
  • Comply with all Privacy Legislation requirements
  • Ability to provide teaching and mentorship in Residential and Community clinical settings.
  • Competent in the use of Microsoft Office products in particular word processing, spreadsheets and presentation software
  • Ability to implement change management initiatives and projects as required to support the organisation in achieving compliance with legislative requirements
  • Current drivers’ licence
