As a Resthaven Coordinator Community Services, you’ll experience the satisfaction of supporting older people to remain living in their own homes, as you interact one-on-one with them to develop and manage care plans.

Western Community Services, located at Woodville,have a full-time fixed-term role (12 month contract) available for the candidate who is looking to take the next step in their Aged Care career.

Key Responsibilities:

• Coordinate referrals to alternative services when required, develop service plans, link clients with resources, monitor, evaluate and review the provision of services to individual clients and advocate for clients.
• Negotiate and inform clients of the cost of goods and services and develop individual client budgets in line with the level of funding.
• Help clients achieve wellness and autonomy through advocacy, comprehensive assessment, planning, communication, health education and engagement, resource management, risk mitigation, service facilitation, and use of evidence-based guidelines or standards.

Essential Criteria:

• A qualification in health, human services or community services and good health literacy skills
• Experience in assessment, referral, and creation/coordination of human services
• Excellent time management skills and the ability to balance competing priorities of individuals’ expectations against resource levels of the program and communicate these to clients.
• An understanding of issues for older people, their families, and carers
• Current Drivers' licence
