We are seeking two Care Finder Advisers to join our team:

Care Finder Adviser - Mallee Coorong
Supporting clients across the Mallee Coorong region (Murray Bridge, Meningie, Lameroo, Pinnaroo and surrounding communities)

Care Finder Adviser: Upper South East
Supporting clients across the Upper South East region (Naracoorte and surrounding communities).

These roles deliver specialist and intensive assistance to help people understand and access aged care services, while connecting them with other relevant supports within their local community.

The successful applicants will have the ability to build and leverage strong local networks, advocate for clients, and work collaboratively with service providers to support individuals to navigate and access the services they need.

The Role:

• Undertaking assertive outreach to identify and engage people who require support
• Supporting clients to interact with My Aged Care and access assessment services
• Assisting individuals to navigate and connect with aged care, health and community support services
• Building and maintaining strong relationships with local service providers and community networks
• Advocating for clients and supporting them to access appropriate services

Essential Criteria:

• Certificate IV in Case Management, Client Care Coordination, Community Services or a related qualification
• Experience in assessment, referral and coordination of human services
• Demonstrated ability to work independently across regional communities
• Current driver's licence (essential)
• Willingness and ability to travel throughout the allocated region
