Bring your passion, experience and knowledge of case management based in the community services arena coupled with your excellent communication skills working with older people to improve their quality of life. The Care Partner - Commonwealth Home Support Packages role is located at our Northern Community Services team in Elizabeth, and is a full-time, 12 month contract opportunity.

Responsibilities

  • Demonstrate service model experience focusing on the delivery of person-centred services and care
  • Ensure compliance with CHSP programs and have sound knowledge and understanding
  • Sound understanding of Assistive technology and home modification funding
  • Work with older people to develop personalised care plans
  • Comprehensive understanding of care planning documentation & reviewing of provision of services
  • Work as part of a broader team and assist the manager with ongoing review and evaluation of programs
  • Provide support to home support workers

Essential Criteria

  • Appropriately trained aged care workers with relevant experience. The following qualifications are highly recommended:
    • Certificate IV in Aged Care,
    • Certificate IV in Disability
    • Certificate IV in Community Services,
    • Diploma of Community Services (Case Management), or,
    • Diploma of Ageing Studies and services
  • Program Coordination, Team Leader, case management experience
  • An empathy and understanding of issues for older people, their families, and care
  • A commitment to supporting older people to remain at home in the community
  • Demonstrated ability to work in a holistic, person-centred model of care
  • An ability to work with individuals to identify their needs, develop personal goals and develop responsive packages of services
  • An ability to work effectively and cooperatively as a member of the Community Services team
  • Knowledge and experience of quality systems for evidence-based care with effective written and verbal communication skills
  • A current SA driver’s license
