We are looking for an experienced and professional Care Partner -Relieving for a full-time permanent role. This is an opportunity to join a caring, supportive environment and make a difference to the quality of care we deliver to our Resthaven Community Services clients. 

About the Role:

The Care Partner - Relieving may work across all programs and as an Intake Partner - dependent on the requirements of the site they are based at. They will work with the intake team to onboard individuals to the appropriate program ensuring that approved services are provided as outlined within the individual’s support plan. The role has a focus on supporting individuals to decide on the services they want to receive that will meet their needs

Due to the nature of Community Services operations, the Care Partner - Relieving may be contacted outside of their usual working hours and required to respond to urgent matters impacting business operations, and may be required to participate in the on-call roster at a site as necessary. Care Partner - Relieving will also be required to work at any metropolitan or country Community Services office to relieve in the role of Care Partner Community Services. A current driver's license is essential.

Core Responsibilities: 

  • Demonstrate a goal orientated service model that focuses on the delivery of person centered
  • services and care
  • Ensure compliance with the program guidelines and expectations
  • Demonstrate a sound knowledge and understanding of the Support at Home categories and service types (clinical support, independence and everyday living)
  • Demonstrate a sound understanding of the Restorative and Palliative care pathways, Assistive Technology and Home Modification (AT-HM) funding, ensuring appropriate referral and service delivery within the streams
  • Working with the individual, develop personalized care plans to meet service plan goals within their approved service types, category and budget.
  • Complete comprehensive care planning documentation and reporting as required for all individuals
  • Coordinate, monitor and review the provision of services
  • Action submission of care plan review as indicated by the individuals’ needs
  • Work with the team to ensure rostering and scheduling of services is seamless and efficient.

Essential Criteria:

  • Appropriately trained aged care workers with relevant experience. The following qualifications are highly recommended:

o Diploma of Nursing

o Certificate III in Individual Support (Ageing),

o Certificate III Health Services Assistance,

o Certificate IV in Aged Care,

o Certificate IV in Disability

o Certificate IV in Community Services,

o Diploma of Community Services (Case Management), or,

o Diploma of Ageing Studies and services.

  • Program Coordination, Team Leader, case management experience.
  • An empathy and understanding of issues for older people, their families, and carers.
  • A commitment to supporting older people to remain at home in the community.
  • A commitment to supporting older people to remain at home in the community.
  • Demonstrated ability to work in a holistic, person centred model of care.
  • An ability to work with individuals’ to identify their needs, develop personal goals and develop responsive packages of services.
  • An ability to work effectively and cooperatively as a member of the Community Services team.
