The Care Partner - Support at Home will oversee and assess the services provided to clients, monitoring outcomes against each individual’s service plan, with an aim to optimize a person’s quality of life and achievement of goals.

Located at the Riverland Community Services office in Berri, this is a full-time permanent position. In this role, it is expected you will demonstrate personal responsibility, effective communication, and adaptability while consistently delivering high-quality service aligned with Resthaven’s values. This includes clear and respectful communication, active listening, and providing timely updates.

Selection Criteria

 Essential

• Demonstrated Program Coordination, Team Leader, case management experience
• An empathy and understanding of issues for older people, their families, and carers
• Knowledge and experience of quality systems for evidence-based care
• A commitment to supporting older people to remain at home in the community
• Proven ability to work in a holistic, person centred model of care
• An ability to work with individuals’ to identify their needs, develop personal goals and develop responsive packages of services
• Demonstrated computer literacy
• A current SA driver’s license

Desirable

. The following qualifications are highly recommended:

• Diploma of Nursing
• Certificate III in individual Support (Ageing) or Health Services Assistance
• Certificate IV in Aged Care, Disability or Community Services
• Diploma of Community Services (Case Management)
• Diploma of Ageing Studies and services
