Care Partner Support at Home Community Services
Resthaven are looking for a Care Partner to provide comprehensive coordination and individualized case management and planning in partnership with Support at Home individuals.
Our Fleurieu Community Services team have an opportunity for a Care Partner - Support At Home to join the team in a Permanent Full-time role. This role is based at our Goolwa office.
The Care Partner will work with the intake team to onboard individuals to the appropriate Support at Home, ensuring that approved services are provided as outlined within their Support Plan. Care Management and coordination are primary functions of the Care Partner role and will be optimized by working with the individual, their supporter, as well as the staff providing care and services.
Core Responsibilities Include:
- Demonstrate a goal orientated service model that focuses on the delivery of person-centred services and care
- Ensure compliance with the Support at Home program guidelines and expectations
- Demonstrate a sound knowledge and understanding of the Support at Home categories and service types (clinical support, independence and everyday living)
- Demonstrate a sound understanding of, Assistive Technology and Home Modification (ATHM) funding, ensuring appropriate referral and service delivery within the streams
- Working with the individual, develop personalized care plans to meet service plan goals within their approved service types, category and budget
Selection Criteria – Essential:
- Appropriately trained aged care workers with relevant experience. The following qualifications are highly recommended:

Diploma of Nursing
o Certificate III in individual Support (Ageing),
o Certificate III Health Services Assistance,
o Certificate IV in Aged Care,
o Certificate IV in Disability
o Certificate IV in Community Services,
o Diploma of Community Services (Case Management), or,
o Diploma of Ageing Studies and services.
- An empathy and understanding of issues for older people, their families, and carers
- A commitment to supporting older people to remain at home in the community
- Demonstrated ability to work in a holistic, person centred model of care
- An ability to work with an individual to identify their needs, develop personal goals and develop responsive packages of services
- An ability to work effectively and cooperatively as a member of the Community Services team, with effective written and verbal communication skills.
- Computer literacy familiar with MS Office Suite and electronic client care systems
- A current SA driver’s licence
