Resthaven is seeking an experienced and compassionate Care Partner to provide comprehensive care coordination, case management and support planning for individuals accessing Support at Home services.

Working closely with individuals, their families, supporters and service providers, you will help ensure services are tailored to individual needs, goals and preferences while supporting people to remain independent and connected to their community.

This is a rewarding and varied role that requires strong organisation, sound judgement and the ability to manage multiple priorities while maintaining a person-centred approach to service delivery.

About the role

As a Care Partner, you will work closely with our intake team to support individuals transitioning into Support at Home services. You will coordinate and monitor services, develop personalised care plans and ensure supports remain aligned with approved funding, service requirements and individual goals.

This is a busy role where no two days are the same. You will manage a diverse caseload, work with multiple stakeholders and balance service delivery, documentation, compliance and client outcomes in a fast-paced environment.

Key responsibilities include:

  • Providing person-centred care coordination and case management.
  • Developing personalised care plans that support individual goals, preferences and wellbeing.
  • Monitoring services and budgets to ensure supports remain appropriate and effective.
  • Coordinating referrals and access to services, including clinical supports, independence services, everyday living supports, assistive technology and home modifications.
  • Working collaboratively with individuals, families, carers and service providers to achieve positive outcomes.
  • Maintaining accurate records and ensuring compliance with Support at Home program requirements.

About you

You are passionate about supporting older people to remain living independently in their own homes and bring experience in aged care, nursing, disability or community services.

You are organised, resilient and comfortable working in a fast-paced environment. You can manage competing priorities, coordinate multiple services and stakeholders, and maintain a high standard of customer service and documentation.

You will have:

  • Relevant qualifications in nursing, aged care, disability, community services, health services assistance or a related field, together with experience supporting older people to live independently in the community.
  • A strong understanding of person-centred care and goal-focused service delivery.
  • Demonstrated experience managing a busy and varied workload with competing priorities.
  • The ability to identify needs, develop goals and coordinate responsive support services.
  • Excellent communication, relationship-building and organisational skills.
  • Confidence using Microsoft Office applications and electronic client management systems.
  • Empathy and understanding of the needs of older people, their families and carers.
  • A current South Australian driver's licence.
