Social Coordinator
The Social Coordinator will have a focus on providing contemporary wellbeing and social engagement programs that optimize each individual’s lived experience. The Social Coordinator will support the Care Partners to deliver holistic high-quality care and services that meet individual needs by knowing the individuals’ choices.
Quality Lead
The Quality Lead is a key role that supports Resthaven continuous service improvement. The Quality Lead will be mentoring and supporting team members and will partner with other key roles, to ensure best practices are embedded within the service offering. This role also supports the leadership team with Quality, WH&S and audit framework.
Care Partner - Self-managed
The Care Partner Self-Managed, will assist individuals who choose to self-manage their Support at Home Services with an aim to maximize the choice and control they have over their services and how they are delivered. The Care Partner Self-Managed will provide oversight and support with ongoing care management. This role will work across all community service offices.
