        We’re now seeking a Care Partner Lead  to join our Community Services team at Marion. This is an opportunity to take the next step in your community services or care management career, leading and mentoring a team with empathy, purpose, and professionalism to ensure all services meet program guidelines, budget expectations, and client goals. 

        Responsibilities Include:

        • Supervising, mentoring, and motivating Care Partners to ensure the effective delivery of community services.
        •  
          Conduct performance management, including appraisals and feedback
        •  
          Foster a positive and supportive team environment.
           
        • Ensuring services are delivered in accordance with program guidelines and standards
        • Liaising with clients, families, and community stakeholders.
        •  
          Building and maintaining positive relationships with clients and community members.

        Essential Criteria: 

        • Tertiary qualification in a relevant health-related discipline, Nursing, Bachelor of Social Work or Allied Health, with a current registration with the relevant governing body e.g. AHPRA.
        • Knowledge and experience of quality systems for evidence-based care.
        • Program Coordination, Team Leader, case management experience
        • Empathy and understanding of issues for older people, their families, and carers
        • A commitment to supporting older people to remain at home in the community
        • An ability to work with individuals’ to identify their needs, develop personal goals and develop responsive packages of services
        • Knowledge of budgeting and finance management
        • Computer literacy 
        • An SA Driver's License 
        • The following qualifications are highly regarded: Diploma of Nursing, Certificate III in individual Support (Ageing), Certificate III Health Services Assistance, Certificate IV in Aged Care, Certificate IV in Disability, Certificate IV in Community Services, Diploma of Community Services (Case Management), Diploma of Ageing Studies and services
